Important Instructions

Your Upcomming Surgery

PLEASE READ THESE INSTRUCTIONS CAREFULLY.  IF THERE IS ANYTHING THAT YOU DO NOT UNDERSTAND, PLEASE ASK THE DOCTOR TO EXPLAIN IT TO YOU.  YOU MAY CONTACT OUR OFFICE AT (815) 399-1234.

DO NOT EAT or DRINK ANYTHING (including water) AFTER MIDNIGHT THE NIGHT BEFORE YOUR SURGERY. If you take a prescribed medication for a heart condition, high blood pressure, infection or other illness, please take your medication with a small sip of water.

On the day of your surgery, please dress comfortably.  Wear a sleeveless or short sleeve shirt.

An adult friend or relative must accompany to the office.  YOUR DRIVER MUST WAIT FOR YOU IN THE OFFICE DURING YOUR SURGERY,  take you home after your surgery, and stay with you for a while.  You will not be able to leave the office by yourself or drive yourself home.  IF YOUR RIDE IS NOT IN THE OFFICE YOUR SURGERY WILL BE RESCHEDULED.

If you must cancel your surgery, our office requires at least 24 hours notice.  Rescheduling your appointment may result in a wait of several weeks.  There will be a charge for any appointment that is canceled without al least 24 hours notice.

We will reschedule your surgery if:

  • You have had anything to eat or drink since midnight
  • You have not taken medications as directed
  • You have not brought an adult who can stay with you and take you home
  • You are late for your appointment

We may ask your to arrive early for your appointment to complete necessary forms.
